Candidate Semester Reports
Complete page 3 during the student's dissertation committee meeting. Submit with completed pages 1 and 2 by last week of Fall and Winter semesters, after student/mentor discussion.
Preliminary Exams
Filled out by Prelim Chair after compiling all committee evaluations (template below). Submit to Prelim Coordinator, student, and [email protected], as a cover page for evals, summarizing all feedback and overall outcomes.
Filled out by each Prelim Committee member and sent with Chair Summary to Prelim Coordinator, student, and [email protected]. Evaluations describe detailed feedback by each committee member.
Student Funding Update Form
Submit an update to a student's funding (new award, new shortcode, new award terms or notice, etc.)
Student Seminars (CMB 850)
Submitted each week by assigned weekly evaluators, plus anyone who wants to provide feedback to the student(s) for their talk(s) at the CMB 850 student seminar.
Teaching Requirement
Must be submitted BEFORE teaching to [email protected], to get position approved and inform mentor of time commitment. Credit for teaching requirement is contingent on approval by CMB and lab mentor.
Must be submitted AFTER/at end of the semester to [email protected] by the student's teaching supervisor. Credit for teaching requirement is contingent on a satisfactory evaluation.
Website & Records Update Form
Submit an update to your student, faculty, or alumni website profile, OR give CMB updated information we might need for our records (especially if you have had a change in job/position title, address, contact information, or other important information).
