Sub-Acute and Long-Term Care

Sub-acute care provides continuity of care for patients who no longer require hospitalization, but still need skilled medical care in a rehabilitation facility. Sub-acute rehabilitation is recommended when a patient is not functionally able to return home. During recuperation, patients receive rehabilitation in a skilled nursing facility, where they stay overnight until therapy goals are met.

The U-M Geriatrics Center and U-M Healthcare System partner with several local rehabilitation facilities to streamline transition from hospital to rehabilitation. This coordination results in shorter hospital stays with fewer complications for our patients. Our goal is to minimize readmission and prolonged hospital stays and to ensure our patients are ready to return home safely after rehabilitation.

Continuity of Care

The Sub-Acute Care Rehabilitation Program offers our patients the advantage of continuity of care by their U-M health care providers during transition and following transfer to a partner rehabilitation facility. All patient care in the rehabilitation facilities are managed by U-M physicians and nurse practitioners, which provides several important advantages:

  • Communication between patients’ U-M Healthcare System providers, rehabilitation providers and personal health care providers remains seamless;
  • Rehabilitation providers have immediate access to patients’ U-M Healthcare System medical records, including X-ray and laboratory tests;
  • All care in the rehabilitation facility is integrated into the U-M Healthcare System patient record, thus allowing for seamless care if re-hospitalization is required;
  • Discharge from the rehabilitation facility is closely coordinated with patients’ personal physicians.

Services provided from our team of rehabilitation experts may include:

  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Skilled nursing care

Partnering Rehabilitation Facilities

Partnering rehabilitation facilities are available close to the U-M Healthcare System and in the surrounding communities. Patients may choose to seek rehabilitation care closer to their homes. The following facilities participate in the Sub-Acute Care Program:
