Patient Experience Learning Forum: Cultivating Connections - Engaging Patients in Cardiac Rehab Care

Join us for a dynamic presentation to learn how one care team has engaged patient advisors to cultivate connections, support new patients, and improve care and well-being. 

The November Patient Experience Learning Forum will feature exercise physiologists Diane Perry and Carolyn Palka, and patient advisor Sandy Schick. The speakers will offer their perspectives on how the Cardiac Rehabilitation program has engaged patient perspectives – and former patients! – to improve the care and experience for new patients, and build meaningful connections between their patients. We hope you’ll attend to gather ideas for how you can implement these practices in your own work!
