The Meds Plus clinic is a chronic care clinic designed to meet the needs of patients who have achieved relative stability on their medications but who continue to require follow up in a specialty setting.
This clinic is primarily focused on long term management of recurrent illness with an emphasis on assisting patients with learning effective self-management strategies and coping skills for managing a chronic illness.
The clinic is staffed by a team which includes an Attending Psychiatrist, Nurse Practitioners and a Social Worker. We also provide clinical rotations in the clinic for Resident Psychiatrists, Nurse Practitioner Students and Social Work Interns who are interested in Chronic Illness Care. We utilize a group medical visit model which allows us to provide cost effective care in a group setting. We also provide walk-in visits and crisis intervention on an as needed basis to clinic patients. The Clinic is staffed by a case manager from Community Support and Treatment Services in collaboration with the Washtenaw Community Health Organization (WCHO).
