Becoming a Member of Silver Club
Steps:
- If you would like to come for an in person tour (15 minutes) with a staff person prior to filling out the intake form, you can call 734-998-9352 or email [email protected]. You can also take a virtual tour.
- Complete Silver Club’s Intake Form. This provides important information regarding the potential member’s medical history, emergency contact information, likes/dislikes etc. This also facilitates more meaningful social engagement with your loved one.

- Once the Intake form has been received/reviewed, the Director (Shannon Etcheverry) will contact you to clarify information as needed and set up a free visit.
Silver Club Free Visit
The purpose of the free visit is to determine if Silver Club is a good fit for you/your loved one, as well as for Silver Club staff. Expect the free visit to last approximately 1 hr- 1 ½ hrs. During this visit, your loved one will get an opportunity to meet the staff, other members, and engage in one of our activities. The Director will meet with you privately to discuss specifics of the program, fill out additional paperwork and offer resources.
What to Bring:
- List of medications (including dosages, times of day etc.)
- Power of Attorney paperwork (if applicable)
- Guardianship paperwork (if applicable)
- Do not resuscitate paperwork (if applicable)
- Diagnosis/assessment from Doctor (if available)
Where we Are:
Silver Club is located in the Turner Senior Resource Center 2401 Plymouth Rd, Suite C, Ann Arbor 48105. We are between Key Bank and the Islamic Center, in a 1 story building. Parking is available on the west side of the building. Please look for signs/the door that say ‘Silver Club’. Our door is on the west side of the building, closest to Plymouth road.
